Rising Sun Child Care enrolls on a first-come, first-serve basis only. If you are interested in enrolling your child, please fill out the following forms below.

Rising Sun Child Care Enrolls on a First Come, First Serve Basis Only

Download each form and fill in all the correct details! 

Please print and do not leave blanks as these are state documents.

Bring in, or send your forms to the following address:

Pay the enrollment fee.
$75 per family 

*This fee is due annually January 1st. For any family starting after July 1st, the fee will be prorated for the first 6 months to $37.50, and then due again in full January 1st.
